GMPRG Client Information Release

Below  is the Client Waiver form which is a release of information from a client pertaining to investigations that 

GMPRG will conduct.  This form is to be filled out by the client and signed prior to any investigation.  Please

download the form below, complete it and either email the form to Admin@gmprg-wi.com or give completed

form to a member of GMPRG's Management Team on the day of the investigation. Please be reminded that if

you have not completed the form the investigation cannot be conducted until after the form has been

received.

GMPRG Conifidentiality Policy

Greater Milwaukee Paranormal Research Group (GMPRG) hereby states that it will keep all information obtained

from a client, pertaining to a case, confidential. GMPRG will not release any information pertaining to any client of

GMPRG including, but not limited to:

Name, address, description of property or the client/property owner.  This agreement covers all forms of

communication including, but not limited to: verbal, written, electronic, photographic, video, or audio not

yet released.

Upon the completion of a “Waiver of Release” by the client, only at that time will information of any kind be released.

GMPRG also understands that upon completion of a “Waiver of Release”, certain information will still be considered

confidential including but not limited to:

Client’s name, address, description of property or the client/property owner, unless authorized by the

client/property owner in writing.

Any member that fails to follow the GMPRG confidentiality agreement will have their membership terminated

immediately and GMPRG and/or the client can/will take legal action.

Under no circumstances will any member of GMPRG ever share information with the media without prior authorization

from the GMPRG Founder and also the client in question.
